There are different schools of thought on this one.

I don't use any of the free stuff.

I use Trend-Micro internet security, and then load on Webroot Spyweeper after that.

That has been doing the trick for me.

Kaspersky is one of the better anti-virus programs right now. RustyPS listed good anti spy/malware programs.
Another good one is Adaware.
Best solution is to stop looking at porn.

I haven't used the newest version of Ad-Aware yet, but the older versions are too slow considering most of the time, all it finds are cookies.
